These terms govern the use of this website, pentadhealth.com, and of the Pentad Health text message program described below. Both are operated by Pentad Health, Inc., a corporation organized under the laws of the State of Delaware, United States. Last updated 16 September 2026.

Use of this website

This website is provided for general informational purposes about Pentad Health, Inc. Nothing on it is medical advice, a diagnosis, or a treatment recommendation, and nothing on it creates a provider–patient relationship. Care decisions should be made with a licensed clinician.

The content of this website may be changed or removed at any time without notice. The website is provided on an “as is” basis, without warranties of any kind to the extent permitted by law.

Text message program

Program description

Pentad Health, Inc. operates a text message (SMS) program for patients enrolled in a remote therapeutic monitoring program at a participating clinic. Messages are transactional and care‑related only: appointment reminders, enrollment and account notifications, and daily home‑exercise check‑in reminders asking the patient to open the app and log a session. No marketing or promotional messages are sent through this program.

How to enroll

Enrollment is in person at the clinic. A patient is enrolled by a licensed provider and gives written consent by signing a paper form that includes a separate, unchecked consent checkbox for text messages from Pentad Health, Inc. Providing a mobile number is optional. It is not a condition of receiving care, and it is not required to use any other part of the service; an email address is collected as an alternate means of contact. Signed and dated forms are retained by the clinic as proof of consent.

Message frequency

Message frequency varies. A patient enrolled in the program should expect approximately 5 to 10 messages per month, depending on the care plan set by the clinician and on the patient’s scheduled appointments.

Cost

Message and data rates may apply. Pentad Health, Inc. does not charge for the messages themselves; any charges are those of the recipient’s own mobile carrier and plan.

Opting out

A recipient may cancel at any time by replying STOP to any message. After replying STOP, one confirmation message is sent and no further messages are sent through the program. To rejoin, the patient may ask the clinic to re‑enroll the number, or reply START. Opting out of text messages does not affect the patient’s care or their access to any other part of the service.

A recipient may reply HELP to any message for assistance, or contact Pentad Health, Inc. at Admin@pentadhealth.com or (909) 447-9287, Monday–Friday, 9:00 AM–5:00 PM Pacific Time.

Delivery

Messages may be delayed or may not be delivered for reasons outside the control of Pentad Health, Inc., including network conditions and the recipient’s device or plan. Mobile carriers are not liable for delayed or undelivered messages. Recipients should not rely on text messages for urgent or emergency communication. In an emergency, call 911 or the patient’s clinician directly.

Supported carriers

The program is supported on major United States wireless carriers. Carrier participation may change without notice.

Privacy

Mobile phone numbers and text message consent collected for this program are used only to operate the program. They are not sold, rented, or shared with third parties or affiliates for marketing or promotional purposes. See the privacy policy for details.

Changes to these terms

If these terms change, the revised version will be posted on this page with an updated date. Continued participation in the text message program after a change constitutes acceptance of the revised terms.

Governing law

These terms are governed by the laws of the State of Delaware, United States, without regard to its conflict of laws rules.
